Who This Program Is For

This program is designed for patients who are medically engaged but struggling with follow-through.

Common referrals include patients who:

  • Are starting or continuing medical weight loss or GLP-1 treatment
  • Are preparing for or recovering from bariatric surgery
  • Have cardiometabolic or chronic conditions where behavior matters
  • "Know what to do” but can’t sustain change under stress
  • Feel stuck in cycles of motivation followed by burnout or avoidanc

This program is adjunctive to medical care — not a replacement.

For patients whose primary barriers are internal, such as stress, anxiety, avoidance, overwhelm, or self-sabotaging patterns.

Nutritional Support

For patients who continue to struggle with eating behaviors despite medical or dietary guidance, this pathway focuses on stabilizing the relationship with food.
